Payment in full is due at the time services are rendered. Cash, check or credit card (MC/VISA) are acceptable. A Superbill with required coded information will be provided to you for submittal to your health insurance company for reimbursement.
NOTE: We do not accept Medicare or Champuspatients. (We are happy to see Medicare qualifying patients, but we regret that we cannot file for you, nor will you receive any compensation.)
Please note! Patients are responsible for calling 48 hours in advance to change or cancel new appointments and 24 hours for follow-up appointments.
We reserve the right to charge in full for scheduled consultations which are missed! (excepting emergencies, of course.)
New Patient Packet
Scheduled 30 minute follow-up appointments (telephone) cost $95.
Unscheduled consults, whether by phone or email/fax, for acute/chronic ailments will be charged based upon the time involved ($3/minute).
After hours and weekend calls will be billed as are unscheduled consults, plus an additional $30 surcharge.
All payments for email/telephone new patient appointments must be made by credit card (MASTERCARD or VISA) at the time of the appointment.
